Transcript:
Provide official transcripts showing all previous undergraduate and graduate work with grades and degrees
A note for International Students: SOPHAS only accepts WES-evaluated transcripts. However, the University of Iowa does NOT accept WES evaluated transcripts. Therefore, you MUST send a separate official transcript to University of Iowa Office of International Graduate Admissions.

Personal Statement:
Write a personal statement. Please address the following questions in a two- to three-page essay, typed and double-spaced. Provide equal time to each question, and in your answers use as much detail as possible, and use specific examples:
Describe your specific areas of interest within epidemiology.
Which primary faculty members in the Department of Epidemiology do you think most closely share your areas of interest?
What previous experiences have influenced your decision to pursue a degree in epidemiology?
How do you see yourself using this degree in the five years following graduation?

Recommendation Letter:
Provide three letters of recommendation. These letters are submitted by your letter writers through SOPHAS.

Thesis:
Provide your master’s thesis (for Ph.D. applicants only. Applicants who did not complete a thesis should submit a sample of scholarly writing).
