Transcript:
You would have to submit a notarized translated copy along with the original transcript.
Students are encouraged to apply online. From GSAS guideline: “Please use the cover sheets provided below when mailing us any supporting materials for your application. All materials should be sent to the Office of Admissions, Graduate School of Arts and Sciences, 107 Low Library, 535 West 116th Street, MC 4304, Columbia University, New York, NY 10027.”

Recommendation Letter:
Letters of recommendation should be submitted through the GSAS online application system.
You may submit your online application even if your letters of recommendation have not yet been submitted. Once the letters are received, we will append them to your application.
Three letters of recommendation are required, but the Graduate School accepts up to five letters.
